DRC forms and documents are available for electronic submission and/or download on this page. Additionally, the DRC has paper versions available in the DRC office. If you have questions, need assistance or need a different, accessible format, please contact us as soon as possible.

Request for Accommodation Form

Use this form to request services, but only if you have already completed an intake form and have been approved for services. This form needs to be completed every academic quarter that you need accommodations.

Intake Form

Use this form if it is your first time contacting the DRC for services. Before you complete this form make sure and review our Getting Started page.

Test Proctoring Form

A Test Proctoring form must be completed for each test taken in the DRC. It is the students responsibility to provide instructors with this form in advance of testing days so that appropriate arrangements can be made

Accessible/Alternative Media Forms

If you are approved for Accessible Media accommodations, this form needs to be completed each academic quarter.
