How to Apply
Application for Fall 2016
January 4, 2016 through June 3, 2016
Step 1: Attend an Information Session
You must attend an Information Session to receive information on the application process and sign up for an advisory appointment. Out-of-state applicants should contact us.
Step 2: Apply to Bellevue College for General Admission ($34)
If you are a current Bellevue College student, please proceed to step 3.
After you have received your Student ID #, please proceed to Step 2.
Step 3: Pay Neurodiagnostic Technology Application Fee
Item #: APPL
Quarter: Quarter we are currently in (do not use quarter you are applying for).
Please print a copy for receipt.
Step 4: Complete Application for NDT Program
Step 5: Submit Application to Program
Application should be provided in an envelope with your name and NDT Application printed on it. Application packet should include:
- Application Form
- Application Fee Receipt
- Copy of CPR for Healthcare Provider card (American Heart Assoc.)
- Compass E-Write results
- Program Specifications signed by a licensed healthcare provider and student
- Immunization Requirements
- Certified National Background Check
- A resume and cover letter, citing education and work experience
- One reference letter from a current or pertinent employer or instructor

**NDT Admission Requirements must be post marked by the application deadline. We will not accept faxes.
Step 6: Submit Transcripts to the Evaluations Office
Submit official college/university transcripts to the Bellevue College Evaluations Office. Bellevue College transcripts do not need to be submitted.

Applying for Additional Programs
For students applying to more than one program, please note that you will need to complete an application and pay an additional application fee for each program.
You should also know that each program has unique requirements, procedures, and application periods/deadlines. It is your responsibility to make sure that your application is complete and submitted before each deadline.
