RN to BSN Required Documents

Documents Required for RN to BSN Enrollment:

After acceptance into the program, students are required to provide the following documents

Tuberculosis
(TB) Test
You must submit one of the following as documentation:

Negative Tuberculin Skin Test (two-step PPD test) Result. A copy of two-step PPD test results within the past 12 months (two injections & two readings given 1 – 3 weeks apart). The document must include the size of induration in millimeter (mm) and negative result. If you had a positive PPD test, you must provide documentation of treatment/Chest X-ray and current clearance from physician. This letter must be dated within 6 months

Annual PPD test result & previous year’s test result. A copy of your current annual PPD test and a previous year’s PPD test. If your annual PPD test lapses and it is more than 12 months apart, you will need to complete the two-step PPD test process. The document must include the size of induration in millimeter (mm) and negative result.

Negative QuantiFERON®-TB Blood Test . A copy of QuantiFERON TB blood test result within the past 12 months
Hepatitis B (HepB)You must submit both documents:

A copy of three HepB vaccines. Please note that it takes 5-6 months to complete all 3 series

A copy of HepB Antibody titer (blood test) result after 3 primary series shots were completed
Measles, Mumps, Rubella (MMR) You must submit one of the following as documentation:

A copy of two MMR vaccines

A copy of Measles, Mumps and Rubella titer results (i.e. 3 titer results)
Tetanus, Diphtheria, Pertussis (Tdap) A copy of Tdap vaccine within the past 10 years
Varicella (Chicken Pox) You must submit one of the following as documentation:

A copy of two Varicella vaccines

A copy of Varicella titer result
InfluenzaYou must submit one of the following as documentation:

A proof of annual Influenza vaccination

A signed waiver for students who decline vaccination

* Specific healthcare institutions may require vaccine for all students or may ask students to obtain a medical waiver from physician.
CPR CARD for Healthcare ProvidersA copy of current CPR card for Healthcare Providers

The CPR card must be issued by a person or facility qualified specifically to instruct CPR for Healthcare Providers (American Heart Association) that includes infant, child and adult. Other types of CPR certification will NOT be accepted.
Criminal History Disclosure FormYou must complete the form and submit it.
Background Check ReportStudents must request a report and submit a copy. The instructions will be provided with an acceptance letter.
