The CEO Program, which is part of the Center for High School Programs department, offers a Youth High School Completion pathway for students aged 16-21. The CEO Program has several forms that are required for admission and required by state law. Below you will find all the printable forms for easy access. Applications

  • Submit a CEO Declaration of Intent. This helps staff find your college application.
  • Submit a Bellevue College application. Be sure this only contains the student information for the state college account.
  • Refer to the Bellevue College State College Application Guide [PDF]
    • On slide 10, select First Year.
    • On slide 11, select Academic.

Submission

Submit paperwork to ceo@bellevuecollege.edu. When submitting an official U.S. high school transcript(s), please either (1) request the high school to send directly the official transcript to ceo@bellevuecollege.edu or (2) submit your sealed, unopened official U.S. high school transcript/s to the Center for High School Programs Front Desk located in the U building on the 2nd floor. Inform the front desk that it is for the CEO Program.

  • Photo ID of student with date of birth. Examples include state ID, driver’s license, passport, or green card. If student has none of these, we can accept copy of their birth certificate.
  • Official High School Transcript from the last high school you attended.
  • Exit date on the high school transcript from your last attended school or official withdrawal letter from the current/previous school if applicable. Refer to the CEO Program Forms page.
