The clinics that are in-network for your insurance will have a green circle and check mark beside them and say &#8220;In-Network&#8221;. *Please note that some of the contact information for the clinics listed may be incorrect. If you have this issue, you can enter the name of the clinic into Google search to get the updated contact details. <\/li>\n\n\n\n<li>If you would like to call the clinic to confirm that your insurance is accepted, or you are calling the clinic to make an appointment, please make sure to have your ID card ready so you can provide them the insurance plan information. A term off refers to not being enrolled in classes on a full-time basis per Bellevue College\u2019s guidelines. <strong>Note<\/strong>: For a Medical Reduced Course Load, the two quarters off must be in a row to have insurance coverage.<\/p>\n\n\n\n<p class=\"has-bc-gold-background-color has-background\"><strong>In no event will a student be entitled to or approved for an insured term off for three full terms in the same policy year.<\/strong> An insured term off for vacation purposes will be limited to a maximum of one quarter per school year. A student who has utilized two insured terms off will not be approved for a third insured term off.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Approved Vacation Quarter with below full-time credits as an insured term off<\/strong>: Students are required to keep the LewerMark plan during the approved vacation quarter as the insured one term off with below full-time credits if they have not already used their insured terms off during the policy year. Premium must be paid in full by the beginning of the quarter through Bellevue College.<\/li>\n\n\n\n<li><strong>Requesting an insured term off due to medical necessity:<\/strong> Students may be able to use an insured term off with below full-time credits in a quarter if the insured term off is available to them. Students must satisfy the following attendance requirements prior to utilizing an insured term off due to medical necessity:\n<ul class=\"wp-block-list\">\n<li>A student in their first term of studies in the United States must have met the definition of Eligible Student and have attended classes for at least the first 30 consecutive days following the beginning of the quarter, unless the student was unable to attend classes during such time due to an acute Covered Sickness or Covered injury. If a student suffers an acute Covered Sickness or Covered Injury during the first 30 days of classes, the 30 consecutive day attendance requirement may be waived.<\/li>\n\n\n\n<li>An ongoing, returning or transferring student must have met the definition of eligibility before being considered for an insured term off.<\/li>\n\n\n\n<li>For a Medical Reduced Course Load, the two quarters off must be in a row to have insurance coverage.<\/li>\n\n\n\n<li>If students are ineligible for insured terms due to previous additional terms off, they will be referred to LewerMark to help them with identifying a suitable alternative plan.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Change of Educational Level (COEL) Gap Term:<\/strong> A student may be approved for an insured term off when transitioning from one education level to another within the same school (e.g., from Associate to Bachelor). The student may be required to submit evidence of enrollment to support the request for an insured term off.<\/li>\n<\/ul>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"wp-block-mayflower-blocks-collapse card bg-primary text-bg-primary\n\t\t text-white\"><div class=\"card-header\" id=\"heading_1b09fc35-9c7a-4773-9fdf-a87e08190ef1\"><h3 class=\"mb-0\"><button class=\"btn collapsed text-white\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse_1b09fc35-9c7a-4773-9fdf-a87e08190ef1\" aria-expanded=\"false\" aria-controls=\"collapse_1b09fc35-9c7a-4773-9fdf-a87e08190ef1\"><strong>Exemptions<\/strong><\/button><\/h3><\/div><div id=\"collapse_1b09fc35-9c7a-4773-9fdf-a87e08190ef1\" class=\"collapse \" aria-labelledby=\"heading_1b09fc35-9c7a-4773-9fdf-a87e08190ef1\" data-parent=\"#accordion_3251d058-e5c1-4f44-91ef-6cfb81a6f7d5\" data-bs-parent=\"#accordion_3251d058-e5c1-4f44-91ef-6cfb81a6f7d5\"><div class=\"card-body bg-light text-dark\">\n<p>Students who meet one of the below conditions are required to submit the Insurance Waiver Request form and a copy of &#8216;Certificate of Coverage&#8217; to the <strong>International Education office <mark style=\"background-color:#F2C01E\" class=\"has-inline-color\">before the start date of each quarter.&nbsp;<\/mark>&nbsp;<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Nearly all F-1 students on Bellevue College\u2019s I-20 or J-1 students on Bellevue College\u2019s DS2019 are required to join the Bellevue College-Foundation arranged accident and sickness medical benefit for duration of enrollment at Bellevue College provided they meet the insurance company\u2019s eligibility requirements. Students need to meet one of the conditions below with proof to be exempted from the mandatory enrolment:&nbsp;&nbsp;<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li>Students who are covered by their family member through their U.S. employer-based health insurance plan.&nbsp;<\/li>\n\n\n\n<li>Student whose finances and insurance are covered by a third-party government sponsorship (e.g., SACM).&nbsp;<\/li>\n\n\n\n<li>Students who have obtained coverage through their parent\u2019s insurance plan from their home country which meets or exceeds the benefits offered by the LewerMark plan and is valid for usage in the United States.&nbsp;&nbsp;<\/li>\n\n\n\n<li>Students who are not on Bellevue College\u2019s I-20 or DS-2019 (e.g., concurrent, other non-immigrant visas)&nbsp;&nbsp;&nbsp;<\/li>\n\n\n\n<li>Students who have applied for post-completion Optional Practical Training (OPT) and are no longer enrolled in any classes at Bellevue College.
