In all such cases, authorization is required before the equipment may be taken away from BC facilities and the following procedures must be followed<\/p>\n<h2>Procedures<\/h2>\n<p>Any administrator, faculty member, or classified employee who needs to borrow college equipment should submit a completed \u201cloan of state-owned equipment to employees\u201d form to the department loaning the equipment. The following information must be provided:<\/p>\n<ul>\n<li>employee\u2019s name;<\/li>\n<li>division\/department\/area;<\/li>\n<li>description of equipment, including model number, serial number, and Washington state inventory tag number;<\/li>\n<li>purpose of the loan;<\/li>\n<li>date to be returned.<\/li>\n<\/ul>\n<p>Both the employee and the supervisor must sign the form. Upon return, the issuing department must initial and date the form and give a copy to the employee.<\/p>\n<p>The loaned equipment is to be used solely for college work. All Bellevue College employees should be aware that state-owned equipment is not to be used for personal or private gain, whether on campus or off, per state ethics laws.
