Clinical Onboarding

Please do NOT print double sided or staple your documents. Paperwork should be handed in T208  on or before deadline unless noted otherwise.

Immunizations (TB, flu, Tdap), CPR and background checks expiring during your clinical placement must  be updated at the time of onboarding.  Clinical sites will not accept your paperwork if anything has expired or is about to.

Franciscan Health System (St. Clare, St. Francis, St. Joseph)

STEP 1: Register and complete online learning modules on www.cpnw.org. See attachment for directions.

      You are registering as:

STEP 2: Complete Online Orientation Forms (link through CPNW.org)
STEP 3: Submit completed paperwork to NDT Program Manager

  • E Learning Modules report from CPNW
STEP 3: Submit completed paperwork (including CPNW E-Learning Module Report)  to NDT Program Manager.

**Student must personally submit drug screening results to onboardingdocs@multicare.org
