Clinical Onboarding

Please do NOT print double sided or staple your documents. Paperwork should be handed in T-208 on or before the deadline unless noted otherwise.

Immunizations (Tuberculosis, flu, Tdap), CPR and background checks expiring during your clinical placement must  be updated at the time of onboarding.  Clinical sites will not accept your paperwork if anything has expired or is about to.

Clinical Placements Northwest (CPNW)

Register and complete online learning modules on See attachment [PDF] for contact information. Registration and completion of CPNW modules required for all students prior to first clinical rotation.

  • STEP 2: Review all Allied Health FAQs and Orientations. Submit completed forms to the program manager.

  • STEP 1: Go to the Providence Everett portal and follow instructions under Allied Health (use ‘Allied Health’ as your search criteria.)
  • STEP 2: Submit the following forms to NDT Program Manager
    • Non-Employee Confidentiality Form
    • Code of Conduct Acknowledgement Form
    • Acceptable Use Agreement Form

  • STEP 1: Log into CPNW ( and select the appropriate Swedish location from the site drop down list. Follow instructions to go to Swedish site and complete all orientations and trainings. Complete all forms and forward the completed Data Access Form to the NDT program manager. – Mac users may have difficulty accessing Swedish Forms
