Nearly all F-1 students on Bellevue College’s I-20 or J-1 students on Bellevue College’s DS-2019 are required to join the Bellevue College-Foundation arranged accident and sickness medical benefit for duration of enrollment at Bellevue College, provided they meet the insurance company’s eligibility requirements. Bellevue College has partnered with LewerMark Agency to provide an affordable quality plan while you study at Bellevue College.

- Register your account by clicking the toggle to “I need to create a login”. Enter your date of birth (MM/DD/YYYY) and your ctcLink ID number. Complete setting up your account by creating a unique password and answering security questions.

- CVS MinuteClinic: A $0 copay and visits are covered 100%, provided they fall within covered benefits.
- Scholastic Emergency Services (SES): Coordinates and pays for travel related emergency services, such as emergency medical evacuation, repatriation of remains, and compassionate family visit if a student is hospitalized, etc. Should there be a hospitalization or critical situation that may require an emergency evacuation, repatriation or remains or other reunion service, notify the Lewer Agency to open a case with SES.
- In-AETNA network Urgent Care Clinics: Requires a $20 copay.

- Enter the zip code or city name for where you would like to find a doctor and click “search”.
- Select the medical plan called “Passport to Healthcare® Primary PPO Network” (this is another name for your AETNA Open Choice PPO plan).
- Select the category of service you are looking for (Urgent Care, Specialists, Mental Health, etc). The clinics that are in-network for your insurance will have a green circle and check mark beside them and say “In-Network”. *Please note that some of the contact information for the clinics listed may be incorrect. If you have this issue, you can enter the name of the clinic into Google search to get the updated contact details.
- If you would like to call the clinic to confirm that your insurance is accepted, or you are calling the clinic to make an appointment, please make sure to have your ID card ready so you can provide them the insurance plan information. The name of your insurance plan is called “AETNA Open Choice PPO”.

Important Notes:
Provided full premium is paid on time through Bellevue College, students may be eligible for coverage up to two quarters off during the policy year. A term off refers to not being enrolled in classes on a full-time basis per Bellevue College’s guidelines. Note: For a Medical Reduced Course Load, the two quarters off must be in a row to have insurance coverage.
In no event will a student be entitled to or approved for an insured term off for three full terms in the same policy year. An insured term off for vacation purposes will be limited to a maximum of one quarter per school year. A student who has utilized two insured terms off will not be approved for a third insured term off.
- Approved Vacation Quarter with below full-time credits as an insured term off: Students are required to keep the LewerMark plan during the approved vacation quarter as the insured one term off with below full-time credits if they have not already used their insured terms off during the policy year. Premium must be paid in full by the beginning of the quarter through Bellevue College.
- Requesting an insured term off due to medical necessity: Students may be able to use an insured term off with below full-time credits in a quarter if the insured term off is available to them. Students must satisfy the following attendance requirements prior to utilizing an insured term off due to medical necessity:
- A student in their first term of studies in the United States must have met the definition of Eligible Student and have attended classes for at least the first 30 consecutive days following the beginning of the quarter, unless the student was unable to attend classes during such time due to an acute Covered Sickness or Covered injury. If a student suffers an acute Covered Sickness or Covered Injury during the first 30 days of classes, the 30 consecutive day attendance requirement may be waived.
- An ongoing, returning or transferring student must have met the definition of eligibility before being considered for an insured term off.
- For a Medical Reduced Course Load, the two quarters off must be in a row to have insurance coverage.
- If students are ineligible for insured terms due to previous additional terms off, they will be referred to LewerMark to help them with identifying a suitable alternative plan.
- Change of Educational Level (COEL) Gap Term: A student may be approved for an insured term off when transitioning from one education level to another within the same school (e.g., from Associate to Bachelor). The student may be required to submit evidence of enrollment to support the request for an insured term off.
Students who meet one of the below conditions are required to submit the Insurance Waiver Request form and a copy of ‘Certificate of Coverage’ to the International Education office before the start date of each quarter.

Students need to meet one of the conditions below with proof to be exempted from the mandatory enrolment:
- Students who are covered by their family member through their U.S. employer-based health insurance plan.
- Student whose finances and insurance are covered by a third-party government sponsorship (e.g., SACM).
- Students who have obtained coverage through their parent’s insurance plan from their home country which meets or exceeds the benefits offered by the LewerMark plan and is valid for usage in the United States.
- Students who are not on Bellevue College’s I-20 or DS-2019 (e.g., concurrent, other non-immigrant visas)
- Students who have applied for post-completion Optional Practical Training (OPT) and are no longer enrolled in any classes at Bellevue College. (OPT students are encouraged to contact Lewer directly to arrange a different plan).
- Students who need a plan with additional coverage that meets or exceeds the benefits offered by the LewerMark plan due to personal health reasons.
Note: Per the agreement between Bellevue College and the Lewer Agency, plans offered by the following companies will not be accepted for a valid waiver: HCC, IMG, ISI, ISO, ISP, PGH, PSI (This is a partial list of plans.)
Bellevue College’s F2 and J2 dependents are not required to join Bellevue College-Foundation Arranged International Student Accident & Sickness Medical Benefit Plan; however, the coverage is available for eligible dependents. The F-1 primary needs to submit the a Dependent Insurance Request form to kazumi.hada@bellevuecollege.edu before the start date of each quarter.
