Please note: All applicants to the ADN program must sign the following acknowledgement. Students at any external sites or practicum are required to follow the business, local, state, and federal requirements. This may include vaccination requirements. Bellevue College does not make any guarantees of placement for practicum or external sites to students not fully vaccinated for COVID-19. Practicum facilities and businesses may deny not fully vaccinated students or students without a booster shot(s) access to their site which may result in an inability or delay to fulfill academic requirements. Alternatives to clinical work and site placements may not be available and these students may not be able to finish the program.

Required Documents

The following documents must be submitted in your CommonAPP. These include:

  • Unofficial Transcripts – for verification of prerequisite courses/GPA
  • TEAS Score Report – for verification of TEAS score
  • Qualifying license – for verification of nursing assistant training, medical assistant certification, emergency medical technician or licensed practical nurse.
  • Bellevue College Nursing AAS-T Academic Advisement Report – for verification of course transfer
  • CPNW Clinical Passport – for verification of clinical participation eligibility
  • AHA BLS/CPR Provider Card – for verification of clinical participation eligibility

Unofficial Transcripts

Unofficial transcripts are requested from any colleges you have taken courses from, including Bellevue College. Usually they can be immediately downloaded or sent to the applicant’s email within a few days.

Unofficial Transcripts are uploaded to the application in the Academic History > Colleges Attended section of CommonAPP.

  • Make sure to submit a transcript, not a course history or degree audit.
  • Do not send any screenshots or photographs of transcripts.
  • Official transcripts sent to BC Enrollment will not fulfill this requirement. Applications without attached unofficial transcripts will not be reviewed.

TEAS Score Report

TEAS Score Reports are given after you have taken the TEAS Test. You can download your score report from ATI Testing.

TEAS Score reports are uploaded to the application in the Program Materials > Documents > Test Score Report section of CommonAPP.

  • Make sure to send your score report, not a screenshot of your score or test scores within Degree Audit.

Qualifying license

All applicants to the ADN program must first complete Nursing Assistant training and/or certification as Medical Assistant-Certified (MA-C), or Emergency Medical Technician (EMT) or Licensed Practical Nurse (LPN) before applying to the program

The Certification/Training Certificates of Completion are uploaded to the application in the Program Materials > Documents > Certification section of CommonAPP.

  • NAC Certification is required for applicants who have completed their training at a for-profit location instead of a community/technical college.

Bellevue College Nursing AAS-T Academic Advisement Report

Applicants will attach their Bellevue College Academic Advisement Report to show that completed courses at other colleges have been transferred.

CPNW Clinical Passport

The CPNW Clinical Passport PDF is filled out by applicants to show that they are ready for clinical participation. The Passport and a tutorial for completing the passport have been provided by CPNW.

CPNW Clinical Passports are uploaded to the application in the Program Materials > Documents > Health Records section of CommonAPP.

  • Fill out all information completely in the Clinical Passport before saving or uploading it to the application, as changes made after uploading it will not be saved.
  • To prevent possible loss of information in your form fields, save the Clinical Passport as a flattened/non-editable PDF before uploading it.

AHA BLS/CPR Provider Card

Applicants must provide an unexpired BLS/CPR Card from the American Heart Association. The BLS/CPR ecard can be claimed by various methods on the AHA website.

AHA BLS/CPR Provider Cards are uploaded to the application in the Program Materials > Documents > Health Records section of CommonAPP.

  • The Renew by/Renewal date is the expiration date of the card. If it is past the renewal date, your card is expired and you are required to renew your training.

Military Paperwork

Applicants who were part of the military and wish to qualify for points as a former military member or a medic/corpsman must provide documentation to show their status.

Military documents are uploaded to the application in the Program Materials > Documents > Military Papers section of CommonAPP.

  • Examples of military paperwork include:
    • Honorable military discharge paperwork
    • Current paperwork showing status and job/rank

Updated Unofficial Transcripts (for reapplying students)

Applicants reapplying to the ADN program may have retaken or taken additional classes before applying again. Since CommonAPP allows only minimal changes between applications, they may be unable to attach an updated unofficial transcript within the Academic History section. Applicants in this situation may attach an updated transcript containing the updated class/classes within the Program Materials section.

Updated Unofficial Transcripts are uploaded to the application in the Program Materials > Documents > Certification section of CommonAPP.

  • Students who are applying for the first time should not attach any transcripts within Program Materials.
  • Reapplying students should try to update their unofficial transcripts in Academic History first.
  • Reapplying students should only submit updated unofficial transcripts within Program Materials if they cannot update them within Academic History.
    • Only submit the transcript with the updated classes. Do not include all transcripts, as they would already have been submitted within Academic History.
