Makeup Test Request Form for Instructors

Please observe the following guidelines for allowing makeup tests for a student who may not be able to take a test/exam at the scheduled class time because of an illness or other extenuating circumstances. Under these circumstances, an instructor may submit a request for the Placement and Testing Services to proctor the test for the student.

Guidelines

A student may not be able to take a test/exam at the scheduled class time because of an illness or other extenuating circumstances. Under these circumstances, an instructor may submit a request for the Placement and Testing Services to proctor the test for the student. To make a request, please follow the process outlined below:

  1. Complete and submit this Request Form to the Placement and Testing Services (Test Center).
  2. Instructor is responsible for arranging a suitable time for the student before submitting the request.
  3. Request must be submitted no less than 24 business hours before requested test time.
  4. The Associate Director or other testing personnel will review the request and respond by email as soon as possible.
  5. If approved, Placement & Testing Services will be responsible for administration and collection of exam.

Makeup test proctoring is available during the timeframes listed below. The test time limit must fit entirely within these hours. Please note: We continue to offer makeup exam service through the breaks between quarters, including August-September.

  • Mondays from 9 a.m. to 12 p.m.
  • Wednesdays from 1 p.m. to 4 p.m.
  • Thursdays from 11 a.m. to 2 p.m.

Fall Quarter 2026 Closures:

  • Tuesday October 13: Offices Closed
  • Wednesday November 11: Offices Closed, holiday
  • Thurs-Friday Nov. 26-27: Offices Closed, holiday

Added test proctoring days due to closures:

  • Tuesday November 10: 9:00 a.m. to 2:00 p.m.
  • Tuesday November 24: 9:00 a.m. to 2:00 p.m.

FINALS Extended hours/Added Days

  • Thursday December 3: from 11 a.m. to 4:00 p.m. extended hours this day due to upcoming finals
  • Friday December 4: from 9:00 a.m. to 12:00 p.m. added day due to upcoming finals
  • Monday December 7: from 9:00 a.m. to 2:00 p.m. extended hours this day
  • Tuesday December 8: from 9:00 a.m. to 2:00 p.m. (added day due to Finals)
  • Wednesday December 9: 12:00 to 4:00 p.m. extended hours this day
  • Thursday December 10: 11:00 a.m. to 4:00 p.m. extended hours this day due to Finals
  • Friday December 11: 9:00 a.m. to 1:00 p.m. (added day due to Finals)
  • Monday December 14: 9:00 a.m. to 1:00 p.m.
  • Proctoring will be available in the usual hours, throughout the break after finals for any lingering makeup exam

Please Note

The Test Center does not have the capacity to administer an exam to an entire class.
